Step 3 – Insurance Renewal Request

  • I have received and reviewed the Boomers/Best Doctors SUMMARY OF BENEFITS for the policy period commencing January 1, 2021.

    Payment for Membership Renewal Dues must be received no later than ____________ at 5:00 PM. You will receive an email confirmation from our Insurance Broker within 48 hours detailing the payment of your insurance premium.

  • Email is our main method of communication with our members. Lots of things change in a year, so please re-confirm your email for our records.
  • In the case of a couple or partner, please add the second member's name also requesting membership renewal/cancellation
  • Please list your land line first (including area code) and then your complete cell phone number. It is imperative that we have a phone number to contact you!
  • For ease of payment, couples/partners may elect to have both members invoiced on the same Invoice. The Invoice will be sent to the first Member's email listed above.
